OCEAN Referral must be filled out by a physician or nurse practitioner via Southlake website.
For children with parents acting as decision makers, please ensure ALL legal guardian(s) are agreeable and ideally, present for the assessment. Referrals will be declined should one or more of the legal guardian(s) not consent for a psychiatric assessment to be done.
