With COVID-19: Child and Adolescent (under age 18) referrals are being accepted from physicians or nurse practitioners for virtual tele-psychiatry assessments consultations via OTN. Select patients can be seen in office.
REFERRAL PROCESS:
Referrals must be filled out by a physician or nurse practitioner. Please fax a referral to Dr. Leung’s attention at 905-830-5977.
For children with parents acting as decision makers, please ensure ALL legal guardian(s) are agreeable and ideally, present for the assessment. Referrals will be declined should one or more of the legal guardian(s) not consent for a psychiatric assessment to be done.
