Cancellation Policy

48 Business Hours notice must be given for any cancellation or else patients/families may be charged. Should a charge be made, future appointments may not be scheduled until payment is received. Two or more missed appointments/late cancellations with outstanding charges will lead to automatic dismissal from the practice. Recognizing that at times, the child may be unable to attend due to unforeseen circumstances, a parent/guardian can attend during the prescribed appointment time (without the child) at initial or follow up appointments, and no charges will be made.Late arrivals of 20min after the appointment time may be considered a no show.

Medication Prescription Policy

Treating children and adolescents with medications requires diligence and care. For this reason, all medications must be prescribed with in person visits. Medications are prescribed until your next scheduled appointment, and should patient/families need to reschedule, it is up to them to ensure that they reschedule during a period when they still have medications. (Be aware that rescheduling may be difficult due to limited availability). Although it is ideal for the child/patient themselves to attend these appointments, if this is not possible, parents can attend without the child once, before the child must attend the next appointment to get medication refills. Prescriptions are not provided over the phone or via faxes.

Uninsured Costs

Dr. Leung charges for the following uninsured services (i.e. services not covered by OHIP):

  • Missed Initial Appointment/Late Cancellation (<48 business hours) $300
  • Missed Follow-Up Appointment/Late Cancellation (<48 business hours) $80
  • Forms/Letter to be filled out: (depend on nature of form) $200/hr.

Dr. Leung does not offer telephone or medical advice over the phone/email. All discussion of clinical matters must be done through an in person appointment.
